NCDP Programs Promoting Diabetes Awareness, Knowledge and Action

WIREDdiabetes

WIREDdiabetes is an NCDP initiative that seeks to improve health outcomes in people with or at risk for diabetes by rewarding them for improving their diabetes health literacy. NCDP has developed an electronic program that will be available online and on kiosks in H-E-B stores, one of the nation's largest independently-owned food retailers. The WIREDdiabetes program will include a targeted rewards program that provides special incentives for users. The first series of kiosks should be available in select H-E-B stores in 2008.

DiabetesXChange.org

Launched in 2006, DiabetesXChange served as the first online community designed to share information about what is working to change diabetes. The Web site acted as a clearinghouse for unique content about diabetes care, treatment and management initiatives in the U.S. This spring, NCDP is excited to announce a new name and partnership for DiabetesXChange - leadinghealthycommunities.com

The Leading Healthy Communities Web site, a repository of best practices in community-based health initiatives, will carry on the Diabetes XChange mission of providing users with an opportunity to share information and learn about the growing number of successful diabetes programs across the country. We are excited about this new home for diabetes resources and hope you will visit leadinghealthycommunities.com soon.

National Diabetes Goal

Some of the nation's leading health advocacy organizations, including the American Diabetes Association, American Assocation of Diabetes Educators and American Association of Clinical Endocrinologists put forth a unified National Diabetes Goal in order to publicly demonstrate our collective will to improve prevention and detection of diabetes. By coming to consensus on a National Diabetes Goal we create a unified, overarching rallying point for the country around diabetes, while still supporting each of our individual organizational missions. Together, we can then leverage the National Diabetes Goal to galvanize public support for changing diabetes® and use the Goal as a platform on which to build and advance strategies that affect positive change.

The National Diabetes Goal Is: By 2015, 45%* of Americans who are at risk for type 2 diabetes will know their blood glucose level and what actions to take.
