Tennessee Public Health Employee Diabetes Education
Program identifies and resolves the knowledge gaps of state health
employees regarding diabetes. Specifically, public health care in
Tennessee has in the past focused primarily on communicable diseases,
instead of on chronic care generally and diabetes care specifically.
Now that the Tennessee Department of Health is shifting focus to Primary
Care, NCDP will use a chronic care model to develop and provide diabetes
education to public health employees.

The Diabetes Information Network of Maryland links people touched
by diabetes to statewide resources for learning about the importance
of adopting positive self-care behaviors.

Employer-Focused Initiatives
NCDP has worked with employers and business groups to identify and
develop employer strategies that encourage better diabetes management
as well as test and demonstrate return-on-investment for employers who
invest in this area. By convening leaders from the business community,
NCDP has helped empower employers to address the impact diabetes on their
health care costs.
